The practicum experience provides the student an avenue to demonstrate a higher levelof case management knowledge and professional development. The on-site practicum project allows students who are new to case management an opportunity to practice in a human services environment and partnership with a practicing Case Manager in the health care setting. The practicum experience is also designed to integrate theory in a reality context of the case management role accomplished through a series assignments, discussions, and journal entries. There will be discussion of issues and trends with relevance to particular areas of case management. The series prepares the learner with essential knowledge needed to function as a case manager.

This course will allow you to use the knowledge gained from your other courses to move through a case from its beginning to its conclusion. You will draw upon your knowledge of texts, community and Internet resources to solve problems that will occur along the course of the clinical practicum. The course will allow you to use your analytical and problem-solving skills, and you are encouraged to be professionally creative in responding to assignments.
